To ensure that each of our clients achieve the 4 wellnesses we start by an in-depth assessment of the client’s needs and wishes. The assessment includes a review of safety at home, medical condition, medications, mental health challenges, emotional challenges, socialization, and other environmental factors that impact client directly or indirectly. Client’s wishes are being heard during the assessment and being taken into consideration. The purpose of the assessment is to provide a better understanding as to what currently works and doesn’t work for the client, identify possible gaps in care/services, assess for compliance with medication, etc. The assessment is used as a tool to understand what is currently happening for the client and what needs to happen next so that the client reaches his/her optimum level of wellness/care. Upon completion of the assessment, our care managers will collaborate with the client to develop the plan of care.
The assessment can be done at home, in a facility, at a hospital, or in our office.
Upon completion of the plan of care, our care managers will coordinate the necessary care on behalf of our clients to ensure the implementation of the plan of care. Most importantly, our care managers are strong in advocacy of behalf of the clients we serve.
This service can help you and your loved one accomplish variety of needed tasks, so that you don’t have to ask yourself “what are the best senior services near me?” We ensure the accomplishment of needed tasks such as:

• Accompany client to key doctor visits
• Routine home visits to ensure all doctor’s orders are followed
• Act as a community liaison between client and local community resources
• Transition to an Assisted Living Facility
• Advocacy during hospitalization and assistance with implementation of the discharge plan
• Access mental health needed services
• Ensuring that providers are delivering effective and efficient care
• Notify families to ensure proper communication
• Coordinate care related to memory disorders

Special Needs Care Management

Property Care Management

Managing a property from a distance is a well known struggle. Our property care management will put an end to this struggle. By hiring our property care management service you will have access to a team of professionals that will treat your property as if its their own through a variety of needed tasks such as :

• Visual inspection of possible property damages such as water leaks, mold, and flooding.
• Monitor lawn care services
• Monitor pool maintenance
• Start all vehicles
• Visually inspect boat to make sure lines are tight and the boat is secure with no damage.
• Check mail box

• Check front door for packages, door hangers, notes etc..

• Booking doctors appointment and ensuring followup in a timely manner
• Running errands
• Grocery shopping
• Staying at your home when a service provider like cable service or technician is scheduled to arrive
• RSVPing to restaurants
• Arranging for house keeping services
• Assist with planning and coordination of a vacation

Educational Care Management

Our educational care management service is specialized and unique as it offers parents access to a trained mental health professional and a certified educational surrogate who can advocate, guide, and support them as they navigate their child’s educational needs. This service aims to facilitate the accessibility to quality educational services which includes, but is not limited to assistance in the development of the child’s individualized educational plan (IEP). This service will also allow parents to seek professional guidance and advocacy related to their child’s education. At RK Care Group we are aware of how challenging it can be to ensure that your child is gaining access to all the services they need as it relates to their education. Therefore, we created this unique service so that you can consult with a trained professional who is knowledgeable of the resources available related to your child’s education.

Telephone Reassurance

Our Telephone Re-Assurance service is geared towards individuals who are socially isolated and in need of weekly friendly phone calls from a caring trained professional. This service is on a monthly membership basis. By becoming a member, you secure yourself a weekly phone call from a caring professional of whom you can share your thoughts and feelings with.

Monthly membership fee is $149.
